On your First Visit please remember to bring:
Clinical Information
- Referral letter from your GP or other Healthcare Professional
- Xrays or scans that you have on film or disc
- A full list of current medications
- A list of any known allergies
- Copies of medical records or other medical test results
- Operation reports from any other surgery
Payment Information
- Medicare card
- Private Insurance Card (if appropriate)
- Other Information related to the insurance
